Fees and Insurance

Fees range from $90 to $210 and are dependent upon the type of service provided.

Dawna Gutzmann, MD is a provider in the Blue Cross Blue Shield PPO network.

Not a provider for Medicare or Medicaid.

Payment is requested at the time of service.  We provide a receipt that can be submitted for insurance reimbursement, if you choose.

For all insurance carriers other than Blue Cross Blue Shield, under special arrangement, we will bill your insurance for you but you remain responsible for any unpaid balance.

Payment is expected for appointments missed or cancelled with less than 24 hours notice.
